Compare your Blue Shield plan options
Access+ HMO plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $0
- Deductible (family): $0
- Primary care office visit: $25
- Specialist office visit: $25
- Infertility services: 50%
Plan highlights:
- Access to providers in our full HMO network
- Predictable costs—fixed copayments for most services
- $0 copay for virtual care (medical and mental health)
- Each member must select a primary care physician (PCP)
- Retail and mail-order prescription drugs
Trio HMO plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $0
- Deductible (family): $0
- Primary care office visit: $25
- Specialist office visit: $25
- Infertility services: 50%
Plan highlights:
- Access to Trio HMO network providers only
- You must live in the Trio HMO service area
- $0 copay for virtual care (medical and mental health)
- Each member must select a Primary Care Physician (PCP)
- Retail and mail-order prescription drugs
PPO plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $250
- Deductible (family): $750
- Primary care office visit: 15%
- Specialist office visit: 15%
- Infertility services: 50%
Plan highlights:
- Available to employees located in the USA
- Freedom to visit any doctor—in or out of our PPO network
- Self-refer to specialists—no PCP referral required
- Urgent and emergency care coverage worldwide
- Retail and mail-order prescription drugs
PPO out-of-area plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $250
- Deductible (family): $750
- Primary care office visit: 15%
- Specialist office visit: 15%
- Infertility services: 50%
Plan highlights:
- Available to employees located outside the USA
- Freedom to see any doctor—in or out of the plan's PPO network
- Self-refer to specialists—no PCP referral required
- Urgent and emergency care coverage worldwide
- Retail and mail-order prescription drugs
PPO 20 plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $250
- Deductible (family): $750
- Primary care office visit: 80%
- Specialist office visit: 80%
- Urgent care: 80%
Plan highlights:
- For Medicare-eligible retirees who do not enroll in Medicare
- Freedom to see any doctor—in or out of our PPO network
- Self-refer to specialists—no PCP referral required
- Urgent and emergency care coverage worldwide
- Retail and mail-order prescription drugs

You must use an Urgent care center affiliated with your doctor’s medical group or Independent Practice Association (IPA).
