Supplemental dental benefits
Ameritas designed supplemental dental benefits specifically for CAPE members enrolled in a CAPE/Blue Shield medical plan. Benefits include preventative fillings; implants; orthodontia for adults and children, including Invisalign®; and more.

The Ameritas supplemental dental benefits are not meant to replace a full dental plan or the Los Angeles County dental plans.

Note: Ameritas also offers an eye care reimbursement plan. This is in addition to the full VSP vision plan described below. For more information, see the 2022 helpful documents

Vision plan
Keep your eyes healthy with VSP® Vision Care. There are no claim forms to complete when you visit a VSP doctor or retail chain affiliate. To find one, go to vsp.com. Or, call (800) 877-7195, Monday through Friday, 5 a.m. to 6 p.m., or Saturday and Sunday, 7a.m. to 5 p.m., Pacific time.
Survivor Life Insurance
The CAPE Benefit Trust now offers life insurance through Symetra Life Insurance Company. Life insurance is important. Prepare for the unexpected by making sure your beneficiaries receive benefits in the event of a tragic loss.

This life insurance is available only to active Los Angeles County employees enrolled in a CAPE/Blue Shield medical plan. For complete eligibility details, contact your dedicated CAPE Benefit Trust Customer Service team.

The survivor life insurance benefit amount is $20,000. As part of this insurance, Symetra also offers support services for beneficiaries who have experienced a loss, travel assistance, and more.

Don’t forget your beneficiaries. You must complete and return the beneficiary designation form below if you want to designate a beneficiary to receive your $20,000 survivor life insurance proceeds.

Voluntary short term disability insurance
Even though no one likes to think about getting sick or sustaining an injury, almost everyone is sure to get medical insurance – just in case. But medical insurance is only designed to cover all or most of the healthcare costs an illness or injury brings.

As a County employee, you are not eligible for the State short-term disability plan. What happens to your paycheck if you can’t work for a while and you’ve exhausted your paid time off? If you’ve never thought about that possibility, now is a good time.

The CAPE Benefit Trust has partnered with Reliance Standard Life Insurance Company to offer CAPE/Blue medical plan members voluntary short-term disability insurance.

Identity theft protection
You may have a home security system that alerts you if someone tries to burglarize your house. To help protect you and your family, you need an alert system for your identity. NortonLifeLock identity theft protection is an essential voluntary member benefit.* It includes dark web monitoring, stolen funds reimbursement, and more.
