HMO Gold Trio Plan ($25 copay)
With the HMO Gold Trio Plan, you:
  • Save money with the lowest-cost plan option from Blue Shield
  • Have no deductible
  • Pay a fixed copayment for most covered services like doctor visits, urgent care, and emergency care
  • Have access to a quality network of local doctors, specialists, and hospitals that work closely together to coordinate your care
Plus, if your primary care physician (PCP) is in the Trio HMO network, you can keep your doctor when you switch to this plan.

Important: To enroll in Trio, you and your eligible dependents must enroll in the same plan and must also live or work within the Trio HMO service area. To see if you are within the Trio HMO service area, visit blueshieldca.com/triocheck.
HMO Platinum POS Plan
With the HMO Platinum POS Plan, you:
  • Have the predictable out-of-pocket costs of an HMO plan with some of the flexibility of a PPO plan
  • Choose a primary care physician (PCP) to coordinate and authorize your care under your HMO-level benefits
  • Can see any doctor or specialist in the Blue Shield PPO Network without a referral from your PCP for select outpatient services
HMO Gold Access+ Plan ($40 copay)
With the Access+ HMO® Plan, you:
  • Have no deductible
  • Pay a fixed copayment for most covered services like doctor visits, urgent care, and emergency care
  • Have access to one of the largest provider networks in California, with more than 13,000 primary care physicians (PCPs), 350 hospitals, and 48,000 specialists
PPO Non-Needles Plan
With the PPO Non-Needles Plan, you:
  • Can see any doctor you choose for most covered services
  • Can self-refer to specialists
  • Will usually pay less for services that are provided by PPO Network providers
  • Pay a copayment or coinsurance for most services after you meet the plan's calendar-year deductible
PPO Needles Plan
The PPO Needles Plan is only offered to members assigned to work locations in the Needles, Trona, and Baker areas. Please check with your benefits administrator to make sure you are eligible to enroll in this plan.

With the PPO Needles Plan, you:
  • Can see any doctor you choose for most covered services
  • Can self-refer to specialists
  • Will usually pay less for services that are provided by PPO Network providers
  • Do not have a calendar-year deductible for in-network services
  • Have a calendar-year deductible for out-of-network services, and will pay a copayment or coinsurance for these services after you meet the deductible

How to find your primary care physician's (PCP's) ID number if you have an HMO plan
You may need your selected PCP’s ID number when you enroll in an HMO plan for the first time.

To find this number, select your plan's Find a Doctor link, and then Primary Care Physician. Once you find your PCP, click on your doctor’s name and then select View details under “Primary Care Physician ID.”
Important notice about urgent care for HMO plan members
If you’re outside of your PCP’s service area – but within California – and you need urgent care, call Shield Concierge first at (855) 599-2657. They will assist you in locating an urgent care center near you that is contracted with Blue Shield.

If you go to an urgent care center that is not contracted with Blue Shield, you may need to pay for the services at the time of your visit. You will then need to complete the Subscriber's Statement of Claim Form and submit it to Blue Shield.

Prescription drug benefits are an important part of your Blue Shield health plan. Understanding your pharmacy benefits can help you save money.

You may save if your medication is a preferred prescription drug. Also, if you take maintenance medications for long-term medical conditions or for chronic conditions such as diabetes, you can have a 90-day supply delivered through our mail service pharmacy. Shipping is free, and you may save on your copay.

You can get a 90-day supply of prescription maintenance drugs from a retail pharmacy. If you take maintenance medications for long-term medical conditions or for chronic conditions such as diabetes, you can obtain a 90-day supply from any participating retail pharmacy in Blue Shield’s pharmacy network.
