Your new member ID card is coming soon!
You will receive a new ID card. To help you manage your health care costs, your new ID card will show your plan’s deductibles and out-of-pocket maximums. At the start of the new year, be sure to present your 2022 ID card to all healthcare and pharmacy providers for any services you receive.

If you’re currently using a printed ID card, remember to discard your old card. Did you know you can access your ID card online at any time? Register on blueshieldca.com/register to view your card 24/7.

If you currently use a digital ID card, you will receive an email with instructions on how to download your new card.

SISC PPO, Tandem, HSA, Minimum Value, and Bronze Plans
These PPO plans are a self-funded plans administered by Blue Shield of California. With a PPO plan, you can see any doctor you choose for most services. You can also self-refer to specialists. You will usually pay less for services that are provided by PPO network providers.

For most services, you will need to meet the plan’s deductible before Blue Shield begins coverage. Once your deductible is met, you’ll pay a copayment or coinsurance for most services and your out-of-pocket costs will be based on whether you see network or non-network providers. You will usually pay less when you use network providers.
Out-of-network services are not covered for preventive services, acupuncture, family planning, arthroscopy, cataract, etc. for PPO members.

Through the BlueCard® Program, PPO members can access urgent care services across the country and around the world. You can receive urgent care services from any provider; however, using the BlueCard Program can be more cost-effective and may eliminate the need for you to pay for the services when they are rendered and submit a claim for reimbursement. Exclusive Tandem PPO benefit

    This app can help you manage stress, anxiety, depression, insomnia, and more. With myStrength, you have access to online resources to help you cope during these challenging times.
Access+ HMO plan
With our Access+ HMO plan you will:
  • Only pay a fixed copayment for most services
  • Be asked to select a primary care physician (PCP) to coordinate your care and refer you to specialists and hospitals within their medical group.
Trio HMO plan
Our Trio HMO plan:
  • Has no deductible, just fixed copays.
  • Is typically more affordable than other plans.
  • Features a select network of local providers who closely coordinate your care.
  • Offers a concierge service to help you with everything from benefit questions to scheduling doctor’s appointments.
Access+ HMO SaveNet plan
Our Access+ HMO SaveNet plan offers the same benefits as the Access+ HMO but at a lower monthly premium. To enroll in the SaveNet HMO plan:
  • You and your eligible dependents must enroll in the same plan and you must also live or work within the SaveNet HMO service area.
  • You must choose a primary care physician (PCP) for yourself and your covered dependents. Each member of your family can choose a different physician and medical group.
Medicare Advantage - Blue Shield 65 Plus (HMO) plan
Blue Shield 65 Plus (HMO) includes Part D coverage, which provides prescription drug coverage, providing you the convenience of having both your medical and prescription drugs covered through one plan.

SISC SOCCCD plan
Your prescription drug benefits are administered through Blue Shield. To access information about your drug coverage log into your Blue Shield account.
