The Blue Shield of California PPO - Accolade plan mirrors your previous plan. Here is how your PPO plan works:
  • You can visit any doctor you choose. Seeing a doctor within the PPO network means you usually pay less for those services.
  • Sometimes you'll need to a specialist. When you do, you can self-refer to specialists -- there is no need to get a referral from your primary physician.
  • Once you've met your plan's deductible, you pay a copayment or coinsurance for most services.
  • With your new PPO plan, you'll have access to Blue Shield's broad network of doctors and specialists. And when you need to find a doctor or check if your doctor is in the Blue Shield network, we can make it easy for you with the Find a Doctor tool.
  • You have exclusive access to an Accolade Health Assistant and nurse. Your Accolade Health Assistant will take the time to get to know you and understand your needs, while partnering with a team of doctors, nurses and benefits specialists to help support you each step of the way and guide you to the right care. Learn more about how Accolade supports you below. (Not available for Blue Shield of CA PPO members living outside of the United States. )

Is the Blue Shield of CA PPO - Accolade the same as my old PPO?

Most of your current plan benefits mirror the new PPO plan option, but with the added benefit of access to an Accolade Health Assistant and nurse, and Blue Shield's broad PPO network. To get more details, see your Summary of Benefits or connect with your Accolade Health Assistant to understand your benefits.

How do I know if my doctor is in the PPO network?

Check to see if your doctor is in the Blue Shield network by using the Find a Doctor Tool. For providers in California, click here. For providers outside of California, access the national network of providers in the Blue Cross Blue Shield network here. Use the prefix SFG when prompted.

Who do I contact about my questions on my PPO coverage and benefits?

Do I have coverage while traveling outside California or the United States?

When you’re outside California or out of the country, you and your family can get care through the BlueCard® and Blue Shield Global Core programs. The BlueCard national network includes more than 95% of providers in the United States. The Blue Shield Global Core network includes providers in 170 countries.
To find a provider in the United States, visit provider.bcbs.com, or call (800) 810-BLUE (2583). To find a provider outside the country, visit bcbsglobalcore.com, or call (804) 673-1177 collect.

When will I receive my new member ID card?

Newly enrolled PPO subscribers will receive a Blue Shield member ID card in the mail before their effective coverage date. Please review your new ID card carefully to make sure all the information is correct.

Does the PPO plan offer wellness discount programs?

Yes. You can get help saving money and living healthier with a wide range of discount programs,1 including Fitness Your Way™. This program gives you access to more than 800 network fitness centers in California and more than 10,000 nationwide for just $25 per month.* The wellness discount programs also include acupuncture and chiropractic services; therapeutic massage; and eye exams, frames, contact lenses, and LASIK surgery.
