Your new ID card should arrive by January 1, 2022
  • The new card will show your plan’s deductibles and out-of-pocket maximums. Be sure to present it to all healthcare and pharmacy providers.

  • If you currently use a printed ID card, remember to discard your old card and use your new one. You can also access your new ID card online at any time by registering at

  • If you currently use a digital ID card, you will receive an email when your new card is ready to view online.

  • Continue to use your current ID card until the new one is available. If you don’t get your new printed ID card by January 1, contact Shield Concierge or Member Services for assistance.
Introducing the Blue Shield of CA PPO - Accolade plan
Effective January 1, 2022, for City and County of SF Employees!
  • See any doctor you choose and pay less for most services by network providers.
  • Access the Blue Shield PPO and Blue Cross Blue Shield's national network.
  • Self-refer to specialists.
  • Pay a copay or coinsurance for most services after the plan deductibles are met.
  • Access an Accolade Health Assistant and nurse for personalized support for your health needs.(Not available for Blue Shield of CA PPO members living outside of the United States. )
Trio HMO plan
Our Trio HMO plan:
  • Has no deductible, just fixed copays.
  • Is typically more affordable than other plans.
  • Features a select network of local providers who closely coordinate your care.
  • Offers a concierge service to help you with everything from benefit questions to scheduling doctor’s appointments.
Access+ HMO plan
With our Access+ HMO plan you will:
  • Only pay a fixed copayment for most services
  • Be asked to select a primary care physician (PCP) to coordinate your care and refer you to specialists and hospitals within their medical group.
PPO plan
With the Blue Shield of CA PPO plan you can:
  • See any doctor you choose, though you usually pay less for services that are provided by PPO network providers.
  • Have the the broad Blue Shield PPO network available to you, including Blue Cross Blue Shield's national network of providers.
  • Self-refer to specialists.
  • Pay a copayment or coinsurance for most services after the plan’s deductible has been met.
  • Access an Accolade Health Assistant and nurse. Accolade is a confidential health and benefits service. (Not available for PPO members that are living outside of the United States)
Urgent Care for Trio and Access+ HMO plan members
For urgent care after office hours and/or on weekends, visit an Urgent Care Center where licensed medical doctors, nurses, and medical personnel are on call for walk-in, non-emergency care.
For Access+ HMO and Trio HMO members, it is important to use an Urgent Care Center in your Medical Group.
To find an Urgent Care Center, click on your Medical Group in the list above, and you will be taken to the listing of Urgent Care Centers within your Medical Group.
If you need prompt medical attention for a non-emergency condition, call your PCP first or visit their website.
