PPO plan
Accolade is a completely confidential health and benefits
service provided through the Blue Shield of CA PPO.
- Exclusively for SFHSS employees enrolled in CA PPO.
- Member can access the Blue Shield PPO and Blue Cross Blue Shields national network.
- Pay a copy or coinsurance for most services after the plan deductibles are met.
- Access the Accolade Health Assistance and use for personalized support and health needs.
- Not available for Blue Shield of CA PPO members living outside of the United States.
With the Blue Shield of CA PPO plan you can:
- See any doctor you choose, though you usually pay less for services that are provided by PPO network providers.
- Have the the broad Blue Shield PPO network available to you, including Blue Cross Blue Shield's national network of providers.
- Self-refer to specialists.
- Pay a copayment or coinsurance for most services after the plan’s deductible has been met.

Access+ HMO plan
With our Access+ HMO plan you will:
- Only pay a fixed copayment for most services
- Be asked to select a primary care physician (PCP) to coordinate your care and refer you to specialists and hospitals within their medical group.

For urgent care after office hours and/or on weekends, visit an Urgent Care Center where licensed medical doctors, nurses, and medical personnel are on call for walk-in, non-emergency care.
For Access+ HMO and Trio HMO members, it is important to use an Urgent Care Center in your Medical Group.
