Providence Access+ HMO plan
With the Access+ HMO® plan, you:
When you enroll in the Access+ HMO plan for the first time, you must choose a PCP for yourself and your covered dependents. Each member of your family can choose a different physician and medical group/IPA.
- Have no deductible – just fixed copayments for most services like doctor visits, urgent care, and emergency care
- Have access to one of the largest provider networks in California, with more than 13,000 primary care physicians (PCPs), 350 hospitals, and 48,000 specialists
- Choose a PCP who will coordinate your care and refer you to specialists and hospitals within their medical group/Independent Practice Association (IPA)
When you enroll in the Access+ HMO plan for the first time, you must choose a PCP for yourself and your covered dependents. Each member of your family can choose a different physician and medical group/IPA.
Providence OptionPLUS HMO plan
Blue Shield developed this plan with Providence Health & Services Caregivers for Providence Southern California employees.
Important: The difference between this plan and Blue Shield's Access+ HMO plan is the provider network. The Providence OptionPLUS HMO plan offers a smaller, specially selected network.
With the Providence OptionPLUS HMO plan, you:

- Have access to a smaller, specially selected network of Providence-affiliated medical groups and primary care physicians (PCPs) and specialists than is available with the Access+ HMO plan

Use our Find a Doctor tool to see if your doctor is in the Access+ HMO Network and to find specialists, urgent care centers, and other providers.
To search for a primary care physician (PCP) by doctor name, select Primary Care Physician. Then, enter your location. Next, click on Doctor Name and enter your doctor's last name. To search for a PCP by specialty, select Primary Care Physician. Then, enter your location, click on Doctor Type, and select a specialty.
When you enroll in the Access+ HMO plan for the first time, you may need your selected PCP’s ID number. To find this number, once you locate your PCP's name in the Find a Doctor tool, click on the doctor’s name and then select View details under “Primary Care Physician ID.”
