Blue Shield HSA Core plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $4,000
- Deductible (family): $8,000
- Primary care office visit: 30%
- Specialist office visit: 30%
- Urgent care center visit: 30%

Blue Shield HSA Plus plan
