- The plan does not require you to go to a dentist within Blue Shield Dental PPO Network, but doing so may result in a lower out-of-pocket cost. PPO participating dentists typically charge less, which means your percentage of the expense will be based on a reduced amount.
- Show your Dental ID card to your dentist to ensure proper claims submission. (Please note that your dental ID number is different from your medical ID number)
- Dependents are not listed on the Dental ID card. If services are accessed by a dependent, the dentist will submit claims in the name of dependent under the employee participant’s ID number.

You received a new dental ID card because of a change in the claims mailing address. There is no change to your dental benefits. Please discard your outdated dental ID card and begin using your new card. To ensure that claims are handled properly, present your new dental ID card to your dental provider’s office at your next appointment.
