A complete plan is a better plan
There's more to good health than eating right, exercising, and getting regular checkups. Taking care of your oral health is just as important.
Tips when using your dental plan:
  • The plan does not require you to go to a dentist within Blue Shield Dental PPO Network, but doing so may result in a lower out-of-pocket cost. PPO participating dentists typically charge less, which means your percentage of the expense will be based on a reduced amount.
  • Show your Dental ID card to your dentist to ensure proper claims submission. (Please note that your dental ID number is different from your medical ID number)
  • Dependents are not listed on the Dental ID card. If services are accessed by a dependent, the dentist will submit claims in the name of dependent under the employee participant’s ID number.
Here's how to get the most out of your Blue Shield PPO dental plan:
  • Visit in-network providers for a deeper discount
  • Take advantage of covered dental cleanings twice a year
  • Don’t delay treating emerging dental issues
  • Use the Find a Dental PPO provider tool to locate a dentist that’s convenient to you
