Access+ HMO plan
In-network out-of-pocket costs (effective 1/1/25):

Primary care office visit: $25
Specialist office visit: $25
Generic drugs: $10/30-day supply
Plan highlights:
- Available to employees located in California
- Access to providers in our full HMO network
- Each member must select a primary care physician (PCP)
- Self-refer to specialists within some medical groups
- $0 copay for virtual care (medical and mental health)
Trio HMO plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $0
- Deductible (family): $0
- Primary care office visit: $25
- Specialist office visit: $25
- Generic drugs: $10/30-day supply
Plan highlights:
- Available to employees located in California
- Access to Trio HMO network virtual and in-person providers only
- You must live in the Trio HMO service area
- $0 copay for virtual care (medical and mental health)
- Enhanced assistance from Shield Concierge
EPO plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $500
- Deductible (family): $1,000
- Primary care office visit: $25
- Specialist office visit: $35
- Generic drugs: $10/30-day supply
Plan highlights:
- Available to employees located outside of California
- Freedom to visit any doctor—in or out of our PPO network
- Low, fixed copayments, just like an HMO plan
- Self-refer to specialists—no PCP referral required
- $0 copay for virtual care (medical and mental health)
Saver PPO plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $1,650
- Deductible (family): $3,300
- Primary care office visit: 20%
- Specialist office visit: 20%
- Generic drugs: $10/30-day supply
Plan highlights:
- Available to employees located in or out of California
- Freedom to visit any doctor—in or out of our PPO network
- Self-refer to specialists—no PCP referral required
- $0 copay for virtual care (medical and mental health)
- Paired with an Health Savings Account (HSA)
SuperSaver PPO plan
In-network out-of-pocket costs (effective 1/1/25):
- Deductible (individual): $3,300
- Deductible (family): $6,000
- Primary care office visit: 30%
- Specialist office visit: 30%
- Generic drugs: $10/30-day supply

- Available to employees located in or out of California
- Freedom to visit any doctor—in or out of our PPO network
- Self-refer to specialists—no PCP referral required
- $0 copay for virtual care (medical and mental health)
- Paired with an Health Savings Account (HSA)

Access care away from home
Away From Home Care (HMO plans)
Students, long-term travelers, and family members living apart can rely on access to health care from Blue Cross Blue Shield plans in select states.
Eligible Trio HMO members who have a serious illness can access non-emergency medical transportation through Call the Car.
Access walk-in health care from registered nurses every day at CVS and Target. (HMO members should check with their doctor's office before visiting a retail clinic.)
Visit urgent care centers when your doctor is not available, saving you time and money compared to an emergency room visit.
Flu shots and other preventive vaccines provided at network pharmacies.
