PPO plan
The Blue Shield PPO plan allows you to:
- See any doctor you choose for most services.
- Self-refer to specialists.
- Usually pay less for services that are provided by PPO network providers.
- Pay a copayment or coinsurance for most services after the plan’s deductible has been met.

High Deductible Health Plan (HDHP)
The Blue Shield High Deductible Health plan allows you to:
- See any doctor you choose for most services.

PPO Select plan*
With the PPO Select plan, you will:
- Have the flexibility to see any doctor in our PPO network.
- Not be covered for services from providers who are not in the PPO network, except for emergencies.
*The following Monterey County facilities are NOT covered under the PPO Select Community Hospital of Monterey Peninsula (CHOMP), Salinas Valley Memorial Hospital, Natividad and George Mee Memorial Hospital.

Trio HMO plan
Our Trio HMO plan:
- Features a select network of local providers, including providers in the Aspire medical group, who closely coordinate your care.
- Offers a concierge service to help you with everything from benefit questions to scheduling doctor’s appointments.
Exclusive Trio HMO plan benefits
- LifeSpring meal deliveryavailable for eligible Trio HMO members only LifeSpring provides chef-inspired, prepared ready-to-heat meals for qualified patients recovering from serious injury. To confirm eligibility, contact Shield Concierge at (855) 747-5800.
