Your new ID card should arrive by January 1, 2022
  • The new card will show your plan’s deductibles and out-of-pocket maximums. Be sure to present it to all healthcare providers.

  • If you currently use a printed ID card, remember to discard your old card and use your new one. You can also access your new ID card online at any time by registering at blueshieldca.com/register.

  • If you currently use a digital ID card, you will receive an email when your new card is ready to view online.

  • Continue to use your current ID card until the new one is available. If you don’t get your new printed ID card by January 1, contact Shield Concierge for assistance at (855) 599-2657.

Tandem PPO plan
Our Tandem PPO plan:
  • Offers the flexibility of a regular PPO plan but is typically more affordable because it provides access to a select network of providers
  • Allows you to see any doctor or specialist − in or out of the Tandem PPO Network − but you will save money when you see a provider in the network
  • Offers Shield Concierge personalized service to help you with everything from understanding benefits to scheduling doctor's appointments

Access+ HMO plan
With the Access+ HMO® plan, you:
  • Have no deductible – just fixed copayments for most services like doctor visits, urgent care, and emergency care
  • Have access to one of the largest provider networks in California, with more than 13,000 primary care physicians (PCPs), 350 hospitals, and 48,000 specialists
  • Choose a PCP who will coordinate your care and refer you to specialists and hospitals within their medical group/Independent Practice Association (IPA)
When you enroll in the Access+ HMO plan for the first time, you must choose a PCP for yourself and your covered dependents. Each member of your family can choose a different physician and medical group/IPA.

Tandem PPO providers
With the Tandem PPO plan, you can see any doctor or specialist in or out of the network. However, you will save money when you see a provider in the Tandem PPO Network.
Important: To verify your doctor’s status in the Tandem PPO Network, call Shield Concierge at (855) 599-2657:

  • In some cases, doctors who have a medical practice at multiple locations may be in the Tandem PPO Network at one location ― but out of the network at another.

  • Blue Shield always tries to show the most up-to-date provider information in our Find a Doctor tool, but this information can change at any time. To check on a doctor’s status, please call Shield Concierge at the number above.
Access+ HMO providers
You may need your selected primary care physician's (PCP’s) ID number when you enroll in the Access+ HMO plan for the first time. To find this number in the Find a Doctor tool, click on your doctor’s name and then select View details under “Primary Care Physician ID.”
