Highlights*

• The Ameritas® supplemental dental benefits are not offered through Blue Shield and are not meant to replace a full dental plan or the Los Angeles County dental plans.
• Benefits include preventative care, fillings, basic root canals, implants, and orthodontia for adults and children, including Invisalign®.
• Ameritas also offers an eye care reimbursement plan, which is in addition to the full VSP vision plan offered to CAPE members.
• To find a provider, visit ameritas.com/find-a-provider. Select Find a Network Dental Provider Online and enter your location. California residents: When prompted to select your network, choose the Ameritas Network found on your Ameritas ID card.

Important: Always show dental providers your Ameritas ID card and not your Blue Shield member ID card.
