In-network out-of-pocket costs (effective 6/1/25):

- Primary care office visit: $20
- Specialist office visit: $20
- Generic drugs: $5/30-day supply

- Access to providers in our full HMO network
- Each member must select a primary care physician (PCP)
- Self-refer to specialists within some medical groups

EPO plan
In-network out-of-pocket costs (effective 6/1/25):
- Deductible (individual): $750
- Deductible (family): $1,500
- Primary care office visit: $20
- Specialist office visit: $40
- Generic drugs: $10/30-day supply
Plan highlights:
- Available to employees located outside of California
- Access to PPO network providers only (nationwide)
- Cost include coinsurance after meeting the deductible
- Self-refer to specialists - no PCP referral required
- $0 copay for virtual care (medical and mental health)
PPO 250 plan
In-network out-of-pocket costs (effective 6/1/25):
- Deductible (individual): $250
- Deductible (family): $500
- Primary care office visit: $10
- Specialist office visit: $10
- Generic drugs: $10/30-day supply

- Freedom to see any doctor – in or out of our PPO network

PPO 550 plan

- Deductible (individual): $500
- Deductible (family): $1,000
- Primary care office visit: $20
- Specialist office visit: $30
- Generic drugs: $10/30-day supply

PPO Savings Plus plan

- Deductible (family): $3,300
- Primary care office visit: 20%
