PPO plan

- Deductible (family): $1,500
- Primary care office visit: $20
- Specialist office visit: $20

- Freedom to visit any doctor - in or out of our PPO network
- Self-refer to specialists—no PCP referral required
- Costs include coinsurance after meeting the deductible
- $0 copay for virtual care (medical and mental health)
- Virtual and in-person mental health benefits

DispathHealth offers in-home, same-day care by a team of medical professionals who can treat more than 40 health conditions. The service is available seven days a week (including holidays), 8 a.m. to 10 p.m., and the cost is typically the same as a in-network urgent care visit.

LifeSpring meal delivery (Trio HMO plan)
LifeSpring provides chef-inspired, ready-to-heat meals for eligible Trio members experiencing a serious illness.

Access care away from home
Away From Home Care (HMO plans)
Students, long-term travelers, and family members living apart can rely on access to health care from Blue Cross Blue Shield plans in select states.
Eligible Trio HMO members who have a serious illness can access non-emergency medical transportation through Call the Car.
Access walk-in health care from registered nurses every day at CVS and Target. (HMO members should check with their doctor's office before visiting a retail clinic.)
Visit urgent care centers when your doctor is not available, saving you time and money compared to an emergency room visit.
